If you have questions about any of these forms, please contact the Office of Planning and Building. Refer to the Planning Board and the Zoning Board of Appeals pages for their forms that may be applicable to your needs.

Proposed construction located within a wetlands buffer zone or a riverfront area, as defined by the Wetlands Protection Act, may require Conservation Commission approval. For further information, email Mary Trudeau at the Belmont Conservation Commission.

Construction requiring the installation of a new or the replacement of an existing sanitary sewer or storm drain requires the filing of a Sanitary Sewer and Storm Drain Connection Permit Application (PDF) form. Please call the Engineering Division if you have any questions regarding this process.
