Dog/Cat License Application

ALL DOGS AND CATS IN THE TOWN OF BELMONT REQUIRE A LICENSE WHICH IS VALID FROM JANUARY 1ST THROUGH DECEMBER 31ST EACH YEAR.  License renewal  begins in December each year and must be completed no later than March 15th of the following year.

IF YOUR DOG/CAT LICENSED IN 2009 HAS DIED OR NO LONGER LIVES IN BELMONT,
PLEASE CALL THE TOWN CLERK’S OFFICE AT 617-993-2600.

LICENSE FEES:
Male Neutered/ Female Spayed -  $12.00 per animal;  $ 9.00 if owner is 60 years of age or older.
Male/Female -                              $37.00 per animal; $34.00 if owner is 60 years of age or older.

The application below is provided for your convenience and can be hand carried to the Town Clerk’s Office or mailed to:  TOWN CLERK, PO Box 56, Belmont,MA 02478.  Please include the license application, license fee, a copy of a current valid rabies certificate and proof that the animal has been spayed or neutered, or a note from the veterinarian stating the reason the animal cannot be spayed or neutered.  If the “rabies expiration date” on last year’s license has not yet passed, you do not need to send the rabies certificate.

If mailing, please enclose a self-addressed, stamped envelope so that the license and tag can be mailed to you. If you have more than one animal, kindly use the reverse side of this form as well. Specify whether Dog or Cat, pet’s name, breed, color and enclose certificates as necessary.

Failure to license your animals will subject you to a $25.00 fine per animal and tickets will be issued starting March 16th.

Remember that licensing is part of our effort to ensure that all domestic animals have timely rabies vaccinations.  
Thank you for your participation. The Health Department offers rabies vaccinations the 1st Saturday in April.

                        1. Current Rabies information and Spayed/Neutered information (if needed)
                        2. Check payable to the Town of Belmont
                        3. Self-addressed stamped envelope
